Why We May Contact You After Your Visit

Sometimes we are not able to verify your insurance information during your Emergency Department visit. When that happens, we may need to contact you after your visit so we can bill your insurance correctly.
Please call Customer Service at 1-800-225-8885 and have this information ready:

• Patient name
• Date of birth
• Name of your insurance plan
• Insurance member ID or policy number

If Your Visit Was Related to a Car Accident

If you were treated after a car accident, we may need additional information before we can bill for your care.

Before calling Customer Service, please:

• Notify the auto insurance company about the accident
• Make sure the driver has reported the accident if you were the passenger
• Complete and return any personal injury protection (PIP) forms sent by the auto insurance company

When you call, please have this information ready:

• Name of the auto insurance company
• Claim number
• Date of the accident
• Name and phone number of the claims adjuster
• Attorney name and contact information, if you have one

If Your Visit Was Related to a Work Injury

If you were treated for a work-related injury, we may need workers’ compensation information to process billing.

Before calling Customer Service, please:

• Report the injury to your employer
• Complete any required first report of injury form
• Get your workers’ compensation claim information

When you call, please have this information ready:

• Name of the workers’ compensation insurance carrier
• Claim number
• Date of injury
• Name, phone number, and fax number of the claims adjuster
